Do you have overbite? Underbite? Try feeling where your teeth meet and come back.

EDIT: I have slight overbite and my upper teeth extend below the upper lip and into the lower lip. And I don't have a huge metal ring pushing my lip up.

You can extend your lower lip to get everything in a position like that pic. Plus, I'd imagine that where it's placed it's extending her lower lip anyways due to placement with the chin. It doesn't look comfortable. At all.

Yeah its possible to get your lips to that position but I have to try and make a very stupid expression for my teeth to be positioned like that. However she looks normal.
Also, why the inside of the ring isn't visible? It has to be about half a cm thick, doesnt it?

The ring/spacer is pushing her lips upwards, see how her mouth is shaped? Shes not smiling or anything but her mouth is arched with the middle pushed upwards, so everything isnt properly lined up anymore. You have to think that they didnt just cut a hole and tore out the skin but rather inserted the ring/spacer which is now pushing that skin in all directions.
